The partnership includes organisations that plan, provide and support health and care services in the borough.

Partners include:

• Local GP practices and Primary Care Networks
• Central London Community Healthcare NHS Trust (CLCH)
• Imperial College Healthcare NHS Trust
• West London NHS Trust
• Chelsea and Westminster Hospital NHS Foundation Trust
• Sobus, on behalf of the wider voluntary and community sector in the borough

The partnership works together to:

• Improve the health and wellbeing of residents and reduce health inequalities
• Join up care across health, social care and community organisations
• Work with local people to empower communities and tackle the wider factors that influence health and wellbeing
