The Hammersmith & Fulham Health and Care Partnership brings together organisations from health, social care, the council and the voluntary sector to support the health and wellbeing of people in the borough.
The partnership works with local communities to help people live healthier lives and access the right support when they need it.
The partnership includes organisations that plan, provide and support health and care services in the borough.
Partners include:
- Hammersmith & Fulham Council
- Local GP practices and Primary Care Networks
- Central London Community Healthcare NHS Trust (CLCH)
- Imperial College Healthcare NHS Trust
- West London NHS Trust
- Chelsea and Westminster Hospital NHS Foundation Trust
- Sobus, on behalf of the wider voluntary and community sector in the borough
The partnership works together to:
- Improve the health and wellbeing of residents and reduce health inequalities
- Join up care across health, social care and community organisations
- Work with local people to empower communities and tackle the wider factors that influence health and wellbeing
The partnership works with residents and community organisations to understand local needs and improve services.
This may include:
- Listening to residents’ feedback
- Working with voluntary organisations
- Supporting local health and wellbeing initiatives
